<p>If you&#8217;ve a traditional indemnity plan and need to make a claim but are not sure how to go about it, then the first thing that you should do is to contact your insurer. In most cases you will find that your insurer has a toll-free telephone number for claims which is manned during normal business hours. When calling you will need to have some basic information to hand, such as your plan number and the name under which the plan is registered. The representative should then be able to simply pull the details of your plan up onto a computerscreen and tell you exactly what you need to do in order to make a claim.</p>
<p>In the past holders of indemnity health plans were required to pay their own medical bills and then complete often lengthy claims forms in order to get reimbursement. Thankfully, this practice is now fast disappearing and increasingly medical facilities are billing insurance companies directly for treatment and the medical facility will then wait to see what percentage of the bill the insurer pays before billing you at a later date for any balance.</p>
<p>If you&#8217;ve a managed care plan and are claiming for something which is quite clearly covered under the plan then, more often than not, you will not need to make a claim at all and the staff at the medical facility where you receive treatment will normally process the necessary paperwork for you. This normally means that they will enter the necessary treatment code into theirpersonal system and, in the first instance, this will tell them whether or not you need to make any immediate co-payment. Thereafter, the details of your treatment, and your co-payment, will be sent directly to your insurer and you won&#8217;t need to take any further action until you receive a letter from your insurer which will detail such things as how much has been applied towards your deductible and whether any balance is due for payment.</p>
<p>In the case of both indemnity plans and managed care plans, if there is any question over whether or not treatment can be carried out under the terms of the plan then it is often the case that the medical services provider will bill you directly at the time of treatment. You are then responsible for discussing the matter with your insurer and seeking reimbursement if the insurer agrees to pay all or part of the bill.</p>
<p>Because of the sheer size of the health insurance marketThis day and the advent of modern computerized medical billing software the days of lengthy claim settlement periods are also thankfully behind us and claims are normally now settled very quickly. <p>Managed care plans were hailed as the inexpensive face of health insurance when they were first introduced but, like most things in life, you get what you pay for and you need to enter the world of managed care with your eyes wide open if you&#8217;re not to be disappointed by the quality of care which you receive for your money.</p>
<p>The principle behind managed care is that it strictly controls costs which, in turn, means that it controls access to care. This means that a health maintenance organization is basically a network of healthcare providers such as Doctorsand hospitals whose services are contracted by an insurer for an concurred fee.</p>
<p>One of the biggest problems with managed health care unfortunately is that it is often the plan holders rather than health care which are managed and there are considerable restrictions placed on the health care which you will receive. For example, you&#8217;ll be required to seek medical treatment within the health care network and that care will usually be controlled by a single doctor, or primary care physician, to whom you will be assigned by the insurer. The result of this is often severely limited access to the health care you want and lengthy waiting times for treatment.</p>
<p>This said, some plans will allow you a degree of freedom of choice and permit you in certain circumstances to seek treatment outside of the health care network. You will however normally be required to pay any costs which your insurer considers to be over and above those normally expected for such treatment within the network.</p>
<p>This does not paint a very rosy picture of managed health care, so why would anyoneselect this rather than traditional indemnity insurance? The simple answer is that the two are very different animals and it is a bit like trying to compare chalk and cheese.</p>
<p>A traditional indemnity health insurance plan is designed to cover you for unexpected accidents and illnesses and does not cover the cost of such things as preventative care and routine medical visits and treatment. By contrast, managed health care focuses on reducing medical bills by keeping you healthy and thus does cover routine medical treatment and preventative treatment, including such things as screening.</p>
<p>Which plan you should choosewill depend very much upon personal preference and your current Stateof health and medical history. For example, if you are young, fit, healthy and single you might well feel that an indemnity plan is a good choice. <p>The answer which you will most often receive is that health insurance plans merely reflect the very high and rising cost of modern medicine and, while this is true, it is also much too easy an answer. To understand the real truth behind today&#8217;s high cost of health insurance you need to startby understanding the basic principles of health insurance itself.</p>
<p>Insurance of any description isn&#8217;t designed to meet normal day-to-day expenses but to meet unexpected expenditure which you would not normally anticipate to meet and which you would nearly certainly not be able to meet without insurance. In other words, it is designed to meet the cost of something which you hope will never happen and so you should also hope to pay for insurance year after year without ever getting anything in return.</p>
<p>Just stop and think for a moment about car insurance. Many people have driven happily for years without ever encountering an accident and have paid thousands of dollars for motor insurance. Others however have been unfortunate enough to be involved in serious road traffic accidents which have resulted in their insurers paying out sums of money which they would never have been able to meet personally without going into bankruptcy. It is however the fact that millions of motorists pay their premiums each year without making a claim against their motor insurance plan which grants the insurer to build up a pool of money from which to pay those claims which do arise.</p>
<p>That&#8217;s all well and good, but what has it got to do with the high cost of health insurance?</p>
<p>The fundamental difference between vehicle insurance and health insurance is that the law requires you to buy the former but does not require you to purchase the latter. In other words, when it comes to meeting motor claims the insurance companies have aMassivepool of money to draw upon because the law requires that anyone who takes a automobile out onto the highway must have insurance. However, the same isn&#8217;t true in the case of meeting health insurance claims because a much smaller number of people are paying into the pool since payment is purely voluntary.</p>
<p>But the lack of a legal requirementto purchase health insurance is only one reason for its high cost. Unlike many other forms of insurance, history shows us that many people only tend to purchase health insurance when they fear that they might need it and are expecting large and growing medical bills. As a result the number of claims in relation to the number of plan holders is high and you have a much smaller number of people paying into a pool from which a much Largernumber of people are claiming.</p>
